Eligibility for the grant:
Any school library or public library in Illinois may apply for the grant.
The school building or public library branch must be registered with the Illinois Readers’ Choice Book Award program selected by May 15. Completing this application does not register you for the award—it is a separate process. If you have not registered for the award program use the links provided below to do so.
Each school building or public library may apply for only one set of books per year.
The books must become part of a centralized, catalogued library collection.
Libraries that received a grant in the previous year are not eligible to receive the same grant in the current year. (For example, if your library received the Caudill set in 2008, you are not eligible to receive the Caudill books in 2009. You may apply for a set of either Monarch, Bluestem, or Lincoln though, if your library also serves these age groups.)

Complete this application in one sitting.
Student eligibility is determined by the number of students in the grades covered by the award.
Estimate the budget amount spent on BOOKS for the students in these grade levels.
For the short answer questions, please be specific and concise. (200 words only.)
Summer contact information will be used for notification purposes.
Shipping address indicates where the books should be shipped to in mid-August.
You must complete this application in one sitting. It will not save.
